Practice
Appointments
Appointments are recommended so that time can be reserved to meet with the doctor. Same-day or next-day appointments are usually available. Emergency appointments are made available as needed. We attempt to see our patients at their convenience and as soon as possible. If appointments need to be cancelled or rescheduled, we ask for as much notice as possible so that the time can be made available to someone else. Our office is staffed Monday through Friday, 8:30-4:30 with the doctor out of the office for hospital patients and volunteer community service on an as-needed basis.
Office Location and Parking
We are located at the northeast corner of Wilshire Boulevard and Witmer Street, on the campus of Good Samaritan Hospital, approximately one-half mile west of Downtown Los Angeles. The entrance to the building is on Witmer Street. There are street meters, parking lots and valet available for parking. Multiple Metro bus lines service our location.
Emergency Care
In life-threatening situations, the paramedics should be summoned first by calling 911. Depending on the severity of the situation, ambulance and/or paramedic transport to the closest open emergency room may be necessary. Once stabilized, transfer may be arranged to Good Samaritan Hospital or St. Vincent Medical Center upon patient request.
Telephone Calls
Patients are encouraged to call with any medical concerns. Messages are taken for the doctor when he is with patients unless the call is an emergency. Call backs are returned as soon as possible. We strive to answer after-hour emergency calls within 15 minutes. For routine non-emergency matters, please call during regular office hours.
Prescription Refills
Prescriptions are based on physical exam and it is best that pharmacy refills be written during office visits. If that is not possible, please call your pharmacy directly and ask them to fax us a request for the refill. For patient safety, we require that all pharmacy orders be in writing. We can refill non-controlled substances if there has been a recent office visit with Dr. Horowitz. For maintenance medications, a two-week back-up supply for disaster and emergency preparedness is recommended. Allow a minimum of one business day to complete refill requests.
Referrals
Our referrals to specialists are not limited to insurance contract affiliations. They are based on quality-of-care concerns. If an in-network specialist is requested, we can review the insurance carrier’s provider list with the patient. Patients are guided through the health care system to help them receive quality medical care.
Insurance and Payment Policies
We accept all insurance plans and are in-network providers for Blue Cross PPO, Blue Shield PPO and Medicare. We see HMO patients but all services are considered non-covered as we are not a contracted provider for any HMO. Payment for copay, deductible and/or non-covered services is due at the time of service. We accept cash, checks and all major credit cards.
Because we contract only with insurance carriers that are fiscally responsible for sustainable family practice, we do not pass additional expense to patients without insurance coverage. Our fee schedule is comparable for everyone, with or without insurance.
